Submit a CSFP Box Request monthly by the 15th for the next month if your request has changed.
CSFP Racial/Ethnic group Participation Data Reporting must be completed annually for the month of April and submitted no later than May 15.
Basic food safety training is required because your agency is handling food. Complete our online Food Safety Training and Assessment OR email a copy of your ServSafe training certificate or proof of other nationally recognized safety training to Sarah Santora.
Participating agencies must maintain a cold storage temperature log because of the perishable cheese provided. Temperatures must be checked and logged at least once per week. Download the log.
